Welcome to the Best Friends Los Angeles adoption program. We request the following information so that we can assist you in the selection of a new dog or cat. This form and a consultation with a Best Friends representative are designed to help you find the dog or cat that is most compatible with your lifestyle.

To be considered as an adopter, you must:

• Be 21 years of age or older
• Have identification showing your present address
• Have the knowledge and consent of your landlord
• Be able and willing to spend the time and money necessary to provide training, medical treatment and proper care of the animal
  

Completion of this application does not guarantee adoption.

If you are interested in giving one of the animals in our program a new home, please complete the appropriate application found below, and someone from our adoption team will contact you once we have received and reviewed your application.

There is a minimal adoption fee which includes spay/neuter, vaccinations & and microchip/ID tag.
